How to Apply

We accept applications from hospice social workers, nurses, and directors who apply to us on behalf of their patients.

Our support is for patients currently receiving hospice care and who are in dire financial need for help in order to afford services that are not covered by insurance, such as rent, heating, and food. We value diversity, equity, and inclusion, and support qualifying patients regardless of their age, ethnicity, gender preference, or background.

Our focus is on providing grants that will increase the quality of life for a patient while the patient is living.

We accept applications directly from the hospice providing care, not from the patient or family. The hospice provider must include an invoice or statement attached from the provider of the service being requested. For example, if this is for help with utility bills, the utility bill must be attached. Applications without an invoice/statement will not be considered.
