How to Apply

Our support helps hospice patients of all ages and backgrounds who are struggling with poverty be able to afford essential services that are not covered by insurance.

Eligibility & Application Guidelines

  • Who qualifies: Adults and children in hospice care with urgent financial need for non-medical services.
  • Who applies: Only the hospice provider may submit applications—not patients or families.
  • When to apply: Requests must be made while the patient is still living (including for final arrangements unless extraordinary circumstances exist).

What’s required

  • A completed application form and bill, statement, quote or vendor website screenshot (if specific item from an on-line vendor) – and mailing address for payment – to support the specific funding request.
  • Application must clearly state other funding resources explored/exhausted (e.g., fuel assistance, family and community support, life insurance), number of adults and children in household, and total household income detailing major expenses (like housing, debt servicing) if not very low income.
  • For final arrangements, documentation showing all other resources have been exhausted (e.g., family help, public aid, life insurance). Assistance for this is capped at $500.

Restrictions

  • We do not accept applications from hospices that have their own foundations or support other hospice foundations.
  • Applications missing required documentation will not be considered.

Funding Process

  • Completed applications are reviewed and voted on within 2 business days.
  • Approved requests are paid directly to the service provider within 24 hours.
  • If funds are no longer needed, notify HHF and return the funds within 24 hours.

Follow-Up Requirements

We would be honored to hear how our support has touched you or your loved one. Within 15 days of receiving assistance, we kindly ask for a short testimonial from the patient or a family member, along with a photo if you are comfortable sharing, and verbal permission to use them.
